ABSTRACT

Objective:To evaluate the influence of a moisturizer containing oat kernel oil for xeroderma and water content of the stratum corneum in children. Methods:From September to December 2022, 30 children with xeroderma were treated in the Dermatology Department of Tongzhou Maternal and Child Health Hospital of Beijing; 13 were males and 17 were females, and the age was 7.33±2.63 years. This was a single-center self-controlled trial. All children applied the moisturizer on the dry skin of the bilateral limbs two time per day for 14 days, and were followed up at 7 days and 14 days. Efficacy was evaluated according to the water content of the stratum corneum, visual scale, xerosis severity scale (XSS), Specified Symptom Sum Score (SRRC), Visual Analog Scale (VAS) and so on. and side-reactions were recorded. Results:After application of the moisturizer, the median of water content in the stratum corneum was 49.00 (33.83, 87.25), 48.84 (32.58, 100.34) at 7 d and 14 d respectively, showing significant increases compared with that at baseline (median 26.51 (16.00, 47.75) ( Z=-3.075, Z=-2.911, P<0.01). The visual scale, XSS, SRRC and VAS showed that compared with the baseline at 7 d, 14 d, the skin dryness and pruritus scores improved significantly ( Z=-4.424, -4.150, -3.943, -4.400; Z=-4.744, -4.409, -4.260, -4.409, P<0.01). Conclusions:The application of this moisturizer containing oat kernel oil could effectively improve skin dryness and the water content of the stratum corneum without serious adverse reactions.

ABSTRACT

Objective:To investigate the clinicopathological characteristics and the risk factors of lymph node metastasis(LNM) of undifferentiated early gastric cancers(EGCs) in elderly patients. Methods:The elderly patients(≥65 years old) who underwent radical gastrectomy with lymph node dissection and were diagnosed as having EGCs in Peking Union Medical College Hospital from January 2010 to August 2019 were included. The clinicopathological characteristics of undifferentiated EGCs(namely signet ring cell EGCs and poorly differentiated EGCs) were compared with those of differentiated EGCs, and the risk factors for LNM of undifferentiated EGCs in the elderly patients were analyzed with logistic regression. Results:Data of 165 elderly EGCs patients were reviewed, including 82 undifferentiated EGCs (11 signet ring cell and 71 poorly differentiated) and 83 differentiated EGCs. The overall LNM incidence was 9.1% (15/165) in elderly EGCs patients, 4.8% (4/83) and 13.4% (11/82) in differentiated and undifferentiated EGCs, respectively. Of all undifferentiated EGCs, the LNM incidence in poorly differentiated EGCs was 15.5%(11/71), and none of 11 signet ring cell EGCs had LNM. Depth of tumor invasion( P=0.019), tumor size( P=0.006), combination of ulceration( P=0.006), depressed gross type( P=0.003) were found to be associated with LNM in elderly undifferentiated EGCs patients under univariate analysis. Multivariate analysis revealed that submucosal invasion( OR=11.98, 95% CI: 1.17-122.84, P=0.037) and tumor size >2 cm ( OR=11.95, 95% CI: 1.88-76.07, P=0.009) were independent risk factors for LNM in elderly undifferentiated EGCs patients. All elderly undifferentiated EGCs patients who met the criteria for expanded indications had no LNM. Conclusion:Submucosal invasion and tumor size >2 cm are independent risk factors for LNM in elderly undifferentiated EGCs patients. The elderly EGCs patients who met the criteria for expanded indications are suitable for endoscopic submucosal dissection.

ABSTRACT

Objective@#To study the effects of endoscopic submucosal dissection(ESD) on long-term quality of life (QOL) and gastric function of patients with distal early gastric cancer (EGC), compared with those of surgery.@*Methods@#Patients with EGC who received ESD or surgical resection in Peking Union Medical College Hospital over 1 year ago were selected to be followed up. QLQ-C30, SF-36, EQ-5D and dyspeptic symptom rating scale were used to evaluate QOL. Five-hour gastric emptying rate was used to evaluate distal gastric function. Electronic gastroscopy was used to observe whether the anastomotic stoma was stenotic. According to the age at resection, 1 to 1 matching was performed between the distal 1/3 gastric ESD (EP) group and the distal subtotal gastrectomy (SP) group, and then the QOL and gastric function between the two groups were compared.@*Results@#Twenty-five patients were included in group EP and group SP respectively. According to QLQ-C30, the scores of cognitive function were 83.3 (83.3, 83.3) in group EP and 83.3 (83.3, 100.0) in group SP (P=0.056). The proportion of patients with symptoms (fatigue, nausea and vomiting, pain, dyspnea, insomnia, appetite loss, constipation, diarrhea and financial difficulties) between the two groups were not statistically different. There was no statistical difference in the scores of EQ-5D and SF-36 between the two groups. According to dyspeptic symptom rating scale, 56.0% patients in group EPhad burning sensation, but only 28.0% in group SP had this symptom (P=0.054). 20.0% of patients in group SPreported nausea, while only 4.0% in group EP had this symptom (P=0.084). Gastric emptying results showed that the proportion of patients with abnormal 5-hour gastric emptying rate was 31.8% in group EP, while there was no abnormal emptying in group SP (P=0.003). Gastroscopy results showed that one patient in group EP had pyloric stenosis, but 5-hour gastric emptying rate was normal. All anastomotic stomas in group Sp were unobstructed.@*Conclusion@#ESD and surgical resection for distal EGC show similar long-term effects on QOL of patients. But the long-term gastric emptying function may decrease after distal gastric ESD.

ABSTRACT

BACKGROUND:Non-operation therapies are commonly used in the treatment of cervical spondylopathy. All of these therapies involve biological mechanics; especially for traction and massage of the cervical vertebrae, they have an obvious property of biological mechanics in rehabilitation of the cervical vertebrae. OBJECTIVE: To probe into the biological mechanics of different rehabilitation methods through comparing the intervention effects between traction and traction combined with massage on cervical spondylopathy.DESIGN: A case-controlled observation. SETTING: Rehabilitation Center of West China Hospital of Sichuan University. PARTICIPANTS: Among the patients who came to the Rehabilitation Center of West China Hospital of Sichuan University between October 2002 and July 2003 for the treatment of spondylopathy, totally 52 patientsmet the criteria for non-operation therapy. They participated in the experiment voluntarily and were randomly divided into 2 groups: the experimental group and the control group, with 26 patients for each. METHODS: ①The patients in the experimental group were treated by traction in combination with massage. Traction was performed once a day,30 minutes each time, 5 times as a course of treatment, and one or two courses of treatment in total. Massage was performed for 8 to 10 minutes each time, once a day, 5 times as a course of treatment, and one or two courses in total. ② The patients in the control group were treated with traction only. Cervical Spondylopathy Therapeutic Effect Rating Scale was adopted to evaluate the effects, three items were selected to set as our scoring criterion, which included clinical symptoms, physical examinations and activities of the daily life (ADL). The improvement index was figured out on the basis of the pre-treatment score and post-treatment score. The improvement index is worked out by the following equation: improvement index = (The post-treatment score-pre-treatment score) / the post-treatment score. The improvement index was evaluated by physicians after the treat ment. Effectual: Clinical symptoms and body symptoms were obviously improved; work, learning, and daily life are not affected. Effective: Clinical symptoms and body symptoms have improved , and work, learning, and the daily life are partially affected. In-effective: There was no improvement in clinical symptoms and body symptoms and showed no effect on the work ,learning, and the daily life. MAIN OUTCOME MEASURES: ① Score of the treatment of cervical spondylopathy of the patients before and after the treatment. ② Intervention effect on the patients after the treatment RESULTS: Totally 52 patients with cervical spondylopathy entered the stage of result analysis with none missing in the midway. ① Comparison of the scores of the treatment of cervical spondylopathy of the patients before and after the treatment: After the treatment, the score in the experimental group was significantly higher than that in the control group [ 16.431 ±3.212,13.147 ±3.036 ( t =4.676, P < 0.01 )], and the improvement index of the patients in the experimental group was significantly higher than that in the control group [0.505±0.163,0.368±0.145 (t=3.860, P < 0.01 )]. ② Comparison of the intervention effects on the patients after the treatment: The effectual rate of the experimental group was higher than that of the control group (80.8%,46.2%).CONCLUSION: The approach of traction combined with massage is superior to simple traction. Different rehabilitation approaches have different properties in biological mechanics. It has showed that the force change is important to therapeutic effectiveness on cervical spondylopathy.
